Requests can be done by going to the office of the Department of Public Health directly or by sending a mail order. When doing the mail option, all of the requirements have to be included including the payment. The state office accepts payments for mail request in the form of check or money order only, mailed-in cash are not accepted. One is expected to wait for days when doing the mail order; days can be shortened to hours if the request was done personally at the office.
